“However, these four scenarios together constitute less than 2% of situations where a CT chest is ordered in COVID-19 cases. Rather than CT findings, it is oxygen saturation that is the key to treatment decisions. Yet, in 95% of the cases, CT scan is a misused tool, often prescribed to rule out pneumonia even in mild cases of COVID-19. There is no point in ordering CT early in the course of COVID-19 because even patients with mild severity may have some ground-glass opacities in the lungs, which do not merit treatment and will resolve on their own,” says R. Aravind, Head of Infectious Diseases, Government Medical College, Thiruvananthapuram.

There is no evidence to support the use of routine multi-phase chest CT in patients with COVID-19 pneumonia. Dr. Guleria said according to data from the International Atomic Energy Agency, one CT scan was equivalent to almost 300 to 400 chest X-rays, which put youngsters at substantial risk of cancer in the long term. A study published in the New England Journal of Medicine in 2007 said based on data from 1991 to 1996, 0.4% of all cancers in the U.S. may be attributable to radiation from CT studies and that the current estimate could be in the range of 1.5% to 2%.

Apart from all this, the risks of transmission and contamination faced by radiology technicians and staff every time a COVID-19 patient undergoes diagnostic imaging, especially in an air-conditioned, closed space, cannot be dismissed.

Dr. Guleria had pointed out recently that he was seeing many mild COVID-19 cases where steroids had stimulated viral replication, causing oxygen levels to drop. There is potential harm associated with the long-term use of steroids in a serious COVID-19 patient. This includes a rise in blood glucose levels, which will need to be carefully managed with insulin to prevent secondary bacterial or fungal infections.

Steroids can turn out to be a double-edged sword if the dosage, timing or duration of the drug goes wrong. Steroids are not mandated for all patients and certainly not for mild patients in the early stages of the illness. The trigger for starting steroids has to be exertional desaturation and not the day of illness.

“Steroids are the most potent weapon we have to combat COVID-19. But the therapeutic window for starting steroids has to be spot-on. Determining the timing, dosage and duration is an art which has to be mastered,” says Dr. Aravind.

One of the main concerns is that we do not want to start steroids too early in the illness when viral replication is happening as it might interfere with the immune system’s natural ability to fight back. We also do not want to miss that critical point when steroids can prevent the immune system from unleashing the cytokine storm, he says.

The WHO guidelines say that steroids may be administered to patients whose resting saturation levels are below 94% and whose respiration rate at rest is over 24 per minute. However, steroids can benefit some patients who are not on supplementary oxygen yet but are showing early indications that they might worsen.

The saturation level has to be checked only after 15 minutes of rest to avoid any fluctuation in the reading.

The experts also stressed the need of checking the oxygen saturation 15 minutes after resting and consult a doctor only when there is a sustained drop at the level, not when there is a one-off dip in the measurement, as the reading can depend on posture too.

 Previously when we used to do prolonged oxygen therapy for people with lung diseases, we have seen people doing fine in oxygen saturation of even 88 per cent. Oxygen saturation of 92 or 93 should not be considered critical. But this is a buffer level when you can seek medical help without panicking,” Dr Guleria said.

ICMR drops plasma therapy from COVID-19 treatment guidelines

Trials had found no significant benefit from the use of plasma

The use of convalescent plasma has been dropped from the recommended treatment guidelines for COVID-19, according to late Monday advisory from the Indian Council of Medical Research (ICMR).

Though a trial by the ICMR on 400 patients last year — called the PLACID trial — had found no significant benefit from the use of plasma 

Several other international trials had also found no benefit from plasma therapy. In fact some experts have said the use of such plasma may have even played a role in the facilitating new worrisome mutations to the virus.

The most recent study published in the British medical journal The Lancet on May 14, reported that in a double blinded trial involving about 5,000 patients who got the treatment in the United Kingdom, again no benefit was found in reducing mortality, or improving patient outcomes.

Crony Capitalism: Meaning and Examples

Crony capitalism is an indicator of the state of the economy where success in business is determined by the mutual relationship between businessmen and government officials.

An Oxfam report 2019, titled “Public good or Private Wealth” revealed that India’s top 10% rich holds 77.4% of the total national wealth. On the other hand, the bottom 60% population holds only 4.8% of the national wealth.

Over 4.9 lakh persons in India developed paralysis between 2000 and 2017 because of oral polio vaccine (OPV), say leading doctors in two reputed hospitals here.

Jacob Puliyel, a paediatrician at St. Stephen’s Hospital, and his co-workers claim their study has shown that “frequency of pulse polio administration is directly or indirectly related to incidence of non-polio acute flaccid paralysis [NPAFP]”.

WHO recommendation

Their study, which calls for judicious use of the OPV schedule to prevent vaccine-induced paralysis, has been published in the International Journal of Environmental Research and Public Health. The Union Health Ministry is yet to respond to the issue.

To monitor the progress of polio eradication, the World Health Organization (WHO) recommends that countries conduct surveillance for cases of acute flaccid paralysis (AFP).

Acute flaccid paralysis

AFP is defined as a sudden onset of paralysis or weakness in any part of the body of a child less than 15 years of age. The surveillance allows nations to detect paralytic poliomyelitis due to wild poliovirus transmission in the population. There are many causes of AFP, so each case needs to be evaluated to find out if the paralysis is due to polio or not. This investigation includes testing stool specimens of all cases for detection of the polio virus.

More than 50,000 AFP cases are investigated in India every year as part of this surveillance system, which has been in place since 1997. In 2009, 741 of these AFP cases in India tested positive for polio. In 2010, only 42 cases tested positive, while in 2011 only a single AFP case tested positive for polio. Not a single AFP case tested positive for polio in 2012, 2013 and 2014. All AFP cases during the last three years have been due to non-polio causes.

“These are all cases of non-polio paralysis, which increased dramatically as polio paralysis was brought down with repeated doses of OPV. This report shows that the rate of paralysis is now coming down where OPV doses have decreased and this is additional proof that paralysis is caused by OPV,” says Dr. Puliyel.

Last case of polio

The last case of polio in the country was reported in 2011, but India, even after it was certified polio-free, maintains its surveillance system in order to pick up any imported cases of polio. In the absence of wild polio transmission, it is expected that AFP cases in India will reduce to an acceptable rate of around two in 1 lakh cases.

“Although surveillance in India has been exemplary, this has not yet materialised,” the report says.

AFP rate in some States is as high as 30 in 1 lakh persons. The present study — using surveillance data obtained from all 36 States and Union Territories — was done to see if the incidence of NPAFP declined with reduction in pulse polio immunisation rounds.

‘High correlation’

The results, however, showed that the number of pulse polio rounds conducted in a State had “high correlation” with NPAFP rate in a State. NPAFP rates in Uttar Pradesh and Bihar were the highest in the country.

“Our study found that NPAFP rate in these States was high in those years when the number of pulse polio rounds conducted was high,” the researchers say.

For instance, there were an additional 47,500 children with paralysis in 2011, which was over and above the assumed NPAFP rate of 2 per 1 lakh cases and the NPAFP rate started to decrease from 2012, when the number of pulse polio rounds decreased.

“From the results, NPAFP rate shows a decline with reduction in pulse polio doses suggesting that OPV vaccinations are responsible for the paralysis. A total of 6.4 lakh children developed NPAFP from 2000 to 2017, suggesting that there were an additional 4.91 lakh paralysed children above the numbers expected to develop NPAFP,” they add.

The report says that “repeated doses of live vaccine virus delivered to the intestine may colonise the gut and alter the viral microbiome of the intestine”.

Also, studies from Finland and Turkey suggest that Guillain-Barré Syndrome (GBS) is causatively associated with OPV vaccination campaigns

“While the mechanism involved is speculative, our findings support the hypothesis that the frequency of pulse polio administration is directly or indirectly related to the incidence of NPAFP. Now that India has been polio-free for over six years, we may be able to reduce NPAFP by further reducing pulse polio rounds,” the report says.

Commending the government for its enormous efforts towards polio eradication, the researchers hope their observation “will help at optimising the dose schedule of OPV administration” to prevent paralysis in vaccinated children.

Dr. Puliyel’s team included Rachana Dhiman and Sandeep Prakash of St. Stephen’s Hospital, and V. Sreenivas of the All India Institute of Medical Sciences.

Polio Vaccination Causes More Infections than Wild Virus

In rare instances, the live virus in oral polio vaccines can mutate and become infectious, causing new outbreaks.

Nigeria, Democratic Republic of Congo, Central African Republic, and Angola have experienced nine new cases of polio caused by the live virus in oral polio vaccines that has mutated into an infectious form, according to statistics released last week (November 20) by the World Health Organization. That brings the global total of these types of infections to 157 for the year, and it means that more children are paralyzed as a result of such vaccine-derived infections than illnesses caused by the wildtype virus, which has affected 107 people this year.

Other countries in Africa and Asia have also reported such vaccine-derived infections, which have the potential to spark new outbreaks. In Africa alone, there are currently a dozen vaccine-derived polio outbreaks, and another was declared in the Philippines last month—the country’s first cases of the disease in more than 25 years, NPR reports.

To finally eliminate the world of polio, global leaders convened last week (November 19) at the Reaching the Last Mile (RLM) Forum in Abu Dhabi, pledging $2.6 billion to the effort. The main impediment is vaccination coverage in certain regions, particularly Afghanistan and Pakistan, the last two countries where polio remains to be eradicated.

While Western countries use an injectable solution of inactivated virus, an oral polio vaccine containing the live, attenuated virus is used for vaccination campaigns in Africa and Asia because it is relatively cheap to produce and easy to administer, requiring just two drops of medicine in the mouth. However, the risk is that the attenuated live virus—in particular, type 2, which is at the root of all current vaccine-derived polio cases, the Associated Press reports—can mutate and become pathogenic. Fortunately, vaccination can protect against such vaccine-derived strains. “The solution is the same for all polio outbreaks: immunize every child several times with the oral vaccine to stop polio transmission, regardless of the origin of the virus,” the WHO states. 

“It’s actually crazy because we’re vaccinating now against the vaccine in most parts of the world,” Vincent Racaniello, a virologist at Columbia University, tells NPR, “not against wild polio, which is confined to Pakistan and Afghanistan.”

Starting in April 2016, public health care workers around the world have made the transition from a trivalent vaccine with types 1, 2, and 3 to a bivalent version without type 2 to prevent such vaccine-derived cases.
